Questions and instructions
Greetings, I am from the National Bureau of Statistics and we are asking your cooperation to participate in a health survey. This survey is funded by the European Union, it is a joint effort of the United Nations and the Federal Ministry of Health, in collaboration with the National Bureau of Statistics of Nigeria. If it is ok with you, I will first ask some questions about the people in your household so that a respondent can be randomly selected. If the selected respondent is at home and agrees to participate, the interview will be conducted privately and will take approximately 40 minutes. Participation is completely voluntary, and the selected participant is not obliged to answer all of the questions if they do not wish and the interview may be terminated at any point.
